The Patient Referral Process
Make a Referral
To refer a patient to The Shores, please call our Welcome Center. A member of our clinical team will walk you through the intake process and coordinate next steps to ensure a safe, timely transition in care.
When you call, please be prepared to share:
- Referring hospital name and phone number
- Referring physician name and contact information
- Patient name, age, diagnosis and level of care requested
- Insurance details and transfer authorization (waived for patients meeting EMTALA criteria)
Welcome Center Hours: Monday to Saturday, 8:00 AM – 7:00 PM
